I started requip xl 4 1/2 weeks ago at 2 mg, increased to 4 mg 2 1/2 weeks ago and again to 6 mg last Wednesday. My MDS had said to take 4 mg at 8am and 2 mg at 2pm in an effort to reduce the possibility of more illusions or hallucinations as well as to help with my energy level for my children afterschool and into the evening. But then on Friday I happened to have an appt w/ the MDS' APRN. We reviewed my symptoms since going on Requip (a few mild headaches, dizzyness upon standing, sore throat, hoarseness, difficulty breathing, cough). Now I know these can be side-effects but some could be PD itself, spring allergies, a virus or other things. The APRN suggested take the 4 and 2 mg pills together at 8 am, and to stick to 6 mg for 3 more weeks rather than increase to 8 mg, precisely why I cannot recall. So now I am confused about whose advice to follow. Does anyone have experience with timing of RXL doses or other insights to offer? I would greatly appreciate some advice and/or support today... -Rose

I usually see better success with the dose split into twice daily with the larger dose being taken in the morning. The split dose seems to give better coverage throughout the day with off times being fewer. I have also seen patients take regular sinemet in the noting to "jump start" their body until the requip xl kicks in. The dose of the xl can be adjusted depending on off time and severity of off times. Hope this helps. Best of health, Mark
