Requip causing extreme imbalance?

Hey- good to see the board back. I've officially been dx'd with an 'unknown' movement disorder after seeing the new neurologist and having yet more rounds of 'normal' testing. I now present with right-sided cogwheel rigidity, bodywide stiffness/increased muscle tone, slowness, and dystonia cramps in all extremeties with my right foot/toes the worst. Doc says he isn't willing to say possible PD because things aren't very typical early symptoms with me, with cramping and extreme stiffness/increased muscle tone. He says he is more leaning toward a rare form of dystonia, but my one-sided cogwheel now he is scratching his head. But, I do definitely have a worse side (not the side I thought- I've always thought left side was worst). I do definitely have a movement disorder, and right now we're treating it with Requip at PD doses as I've had positive response with it back in January.

I'm up to 1.5 mg 3 x day and am now having severe balance issues. I pull hard to my left side, have light dizziness that if I stand too quickly makes me feel nauseous and the room spins. I feel like my left eye is having more focusing and movement difficulties- as it has in the past, but worse. I know I am not sick, no sinus infection, cold or flu, nor do I have allergies- all I can think is it might be this higher dose of Requip? The other side effects I've tolerated so far okay are increased nightmares, mild swelling of my feet, calves, and hands, and mild dropping of blood pressure upon standing too quickly (hypoorthostatic blood pressure- whatever that's called- ha!). I feel like I'm continuing to see improvement with the Requip as I increase the doses weekly, but I'm hoping I can still get better with a bit higher dose- my swallowing and speech are still problematic, and my dystonic cramps- my two biggest things I'd like improved now. My walking is has improved dramatically-faster and more fluid, and rolling in bed is good again, the pain/stiffness in left shoulder nearly gone, and left sided face stiffness is gone too.

Do you think I should stick this out to the next dosage increase (Saturday) or do you think this is my sign that I'm at the highest dose I can tolerate right now for the severity of my symptoms? Or are these side effects (the imbalance issues) more serious than sticking with this Requip even? Doc said if I decide not Requip then he'll switch me to Mirapex.

This trying to find the dose best for me is SO difficult. Thanks for any input!! Connie

Hello again! Well, the imbalance issues were due to a slow progressing head cold- ha! I emailed the doc with some of my side effects and benefits I'm seeing and he said to definitely go to 3 mg 3xday and stay at that until I see him again in August. I'm at 2.5 mg now and can not believe the massive improvements in my walking! I am jogging up stairs now, walk with co-workers at a nearly normal pace, and the stiffness/increased muscle tone in my legs and arms is starting to go down. I definitely notice on/off periods with the Requip where an hour or so before my next dose symptoms start to become more noticable and pick up until an hour or so after I take the medication again. Is that common with Requip- the on/off effect? I know it is with sinemet.

I have more nausea and slight drowsiness now at the higher dose of Requip, but otherwise the side effects don't bug me too much. I will start 3 mg tomorrow and we'll see how it all goes.
